This Notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Who Will Follow This Notice

This Notice of Privacy Practices applies to all of Helen Newberry Joy Hospital & Healthcare Center (“HNJH’s”) service locations and healthcare professionals who provide services at HNJH. HNJH is also part of an organized healthcare arrangement with other Upper Peninsula hospitals and their medical staff. All of these hospitals and their entities, sites, and locations they operate, have notices similar to this Notice.

This notice applies to all of the records of your care generated by HNJH. This notice will tell you about the ways in which we may use and disclose medical information about you.
